Neither the DSM nor the ICD-eleven diagnostic standards require actual sexual activity with a prepubescent youth. The diagnosis can thus be made according to the existence of fantasies or sexual urges even whenever they haven't been acted upon. Alternatively, a one that acts on these urges but experiences no distress with regards to their fantasies
